Please READ CAREFULLY and affirm the following: I have a Medicaid provider number and intend to bill Medicaid directly for this client and will accept $250 as a minimum payment for prenatal and postnatal support only as defined by the MOMCHA Program agreement. If I do not have a Medicaid ID currently, I understand that a Medicaid provider number may pending for me if I have completed an application for such. Therefore, I will not be billing Medicaid as of this application for this job posting. If my Medicaid provider number status changes, I will alert the MOMCHA Program Admin within 24hrs via email at metromommyagency@gmai.com. I have a current CPR certification and ifnot, will complete it before the assigned client is 36 weeks gestation. I affirm that I am available to serve this client prenatally (2 weeks before due date and 2 weeks after due date), postnatally (post birth and up to 6 weeks postpartum) or both. I speak the languages listed in my bio fluently and I affirm that I am able to write UniteUs case notes in English and submit reports as outlined in my MOMCHA PIP Agreement. I have access to reliable transportation. I have the capacity and technology to provide telehealth/virtual services as needed. I affirm that I will communicate with the MOMCHA Program Admin if I am unable to serve a MOMCHA Program client/cannot attend an event with at least 48 hrs notice before the due date/event date so that MOMCHA Program Admin can find a suitable replacement from the MOMCHA Program doula pool if I am unable to secure a MOMCHA Program doula to replace me.
